INVITATION TO BID CFP Project Number: OK56P062501-15 Bid Opening Date: Tuesday, October 25, 2016 Bid Opening Time: 10:00 a.m. Bid Opening Place: McAlester Housing Authority - Office 620 W. Kiowa McAlester, OK 74502 Project Architect: Chase Fetters Hewitt - Architects 4520 S. Harvard, Ste. 180, Tulsa, OK 74135 The Housing Authority of McAlester will receive bids for the 2015 CFP Modernization Project which consists of replacement of bathtubs, shower and tub valves/controls, wall surrounds, and replacement of lavatory traps and drain lines at designated housing units. Work will be performed at various sites throughout the McAlester Housing Authority’s properties in McAlester, Oklahoma. Sealed proposals for this work, including the furnishing of all necessary materials, labor, and equipment, will be received until the above stated time and date in the care of the McAlester Housing Authority. Sealed bids shall be marked “2015 CFP Modernization Bid - DO NOT OPEN”. Plans and specifications, together with all the other necessary form documents for bidders may be secured from the office of the Architect or the office of the Housing Authority.- A certified check or bank draft, payable to the McAlester Housing Authority, U.S. Government Bonds, or a satisfactory bid bond executed by the bidder and acceptable sureties in an amount equal to 5 percent of the bid shall be submitted with each bid. Each bidder shall complete the Form of Bid, the Bid Bond, the Equal Opportunity Certificate, the Non-Segregated Facilities form, the Form of Non-Collusive Affidavit, the Form of Statement of Bidder’s Qualifications, the Bidders Certifications statement and the Bidder’s Previous Participation Certificate, the Section 3 Clause and submit it in the bid package. The successful bidder will be required to furnish and pay for satisfactory performance and payment bond or bonds. Any addenda issued during the time of bidding are to be covered in the proposal and shall become a part of the contract. All related bidding documents are on file at the public Housing Authority’s office, Dodge Reports/Tulsa, Bid News/Tulsa and Southwest Construction News/Tulsa, and may be obtained from the Architect for a deposit of $50 per set, plus non-refundable $20 per set charge when mailing is requested. Upon return of all sets of Plans and Specifications in good condition by unsuccessful bidders within 10 days of bid, the Housing Authority shall refund the deposit in full.
